Hi Mike,

The version of the questionnaire module in http://download.moodle.org/download.php/modules/questionnaire.zip seems to have a version date of 2005062701.

Is there a more up-to-date version available as a zip file?

That should be the latest. The version number in the 'version.php' file is for the database version only.
