only 1 question shows on review

We're using 2.7.X+

A teacher gave a quiz to students, all questions (39) are one one page, but when students finish the quiz they can only review the first question.

so to recap: the teacher can see the total on the grade report page, but reviewing the attempt for both teachers and students has no navigation, students are stuck on the first question, teachers can open up individual questions from the grade report but also can not navigate the whole attempt.

Does the incomplete review page have all the usual stuff in the page footer? If not, then probably a fatal error is occurring, and you should turn on Debugging to find out the error message.

If that is not the problem, then I will try to think what else might be happening.
