How to design this quiz question ?

How to design this quiz question ?

I have 10's of quiz questions of this type (shown in the image).

 (this is a demo sample)

I prefer not to redesign the question (if possible).

As it is not a standard type of questions, what is the way to design it ?

I tried some third party Authoring tools but with no success.

Anyone can suggest !

You could do something quite close to this with the OU's Drag and drop onto image question type.

Alternatively, if you don't want to instal extra plugins, you could put lables like A, B, C in the image, then do the choices underneath, using a Cloze question.
