Hi,

We're currently using version 2.6.5+ (Build: 20140911).

We have one instructor who reports that for multiple Moodle classrooms he receives notifications in which the name and address of the indidividual in the email "from" and/or "reply-to" fields do not match the name of the person in the body of the message who actually performed the activity. This is intermittent, but happens to multiple students as well as to the instructor himself. He forwarded several of these "mismatched" messages to me, and I've confirmed something appears to be amiss. In all cases, it seems that the body of the email shows the name of the correct individual, but the "from" and/or "reply-to" fields are wrong.

I've also confirmed that the individuals all have unique IDs, usernames, email addresses, first names, last names, etc. No apparent discrepancies in records in the relevant database tables. I purged caches and ran mySQL "check table" against every table in the Moodle database.

Here are some of the "mismatched" messages. I would appreciate any clues in determining the cause of this problem.
