It really depends on how you intend to use this Dataform activity.
If you want to allow the user to add more than one address and phone number, you can simply add fields for each set. Assuming that there is a reasonable limit, say 5, you can add fields Address1, Address2, ..., Address5, and Phone1, Phone2, ..., Phone5.
If you want unlimited addresses and phone numbers, you can design the acivity such that the user adds an entry for each address-phone pair.
In any case, you don't really need a Name field becuase the entry author name is taken from the user acount.
If you want to allow editing of the age only once, you can use a field access rule which denies editing of the Age field if it is not empty. I'm not sure, however, that it's a good approach to deny editing of this field. Users may need to change the value in there if they made a mistake and will not be able to.
