I"m using moodle 2.7 the questionnaire version is 2014051200
When adding to the question Type of rate scale N/A column it adds on the left side of the table a column of checkboxes
The Rate question now features extra "not yet answered" radio buttons. This is especially useful in Rate questions with a long list of choices.
