It is possible to add an assignment that is relevant only for teacher?
It is an assignment that is part of the student's course, but the student has nothing to do with it.
For example: an oral test
the student does not assign anything
but the teacher should give his feedback
Ah i see Then please see the documentation Using assignment and the section 2.6 You want to grade work students have done offline. When setting up a regular assignment you simply untick/uncheck all submission options such as online text or file upload so the student doesn't submit anything but the teacher can grade and give feedback.
looking at it a different way, a way that considers pedagogical implications let us role play .

I am a teacher of primary and secondary pupils
I am a teacher of FE/HE
I am a midwife
I am an independent online teacher
We all utilise observation to contribute to an holistic view of the developing learner.
Like Iris, we want to record our observations (this is theoretically based on social learning theory)...so we are not interacting in a way that requires any sort of submission of an artifact from the learner- in moodle- the exception being perhaps the online teacher.
Having read the using assignment page......submission appears to us as a very popular word......across the documentation. We don't want that. In fact we don't want the assignment here....we only need a section/space in the gradebook to input, for example, codes for behaviour...I, the midwife might want to use moodle to train my students in good record keeping and get them to log the outcomes from their centile charts.....I the early years teacher might want to log my pen-portrait (child dev) scores.....I the secondary music teacher might wish to log performance grades and prepare feedback...
sounds to me like a gradebook related item that could enhance practice.
In response to Dawn, I think the difference is:
- if you use an assignment to grade a task done offline then the student can see and has easy access to the teacher's feedback because there is a link to the assignment on the course page (and possibly also their My home page) It's easier to give written feedback as well as the grade.
- If you add a grade as a grade item directly to the gradebook (which is another option, Iris) then the student won't automatically see it unless you tell them to look in the gradebook.
Okaaaaaaaaaay..........I have not been clear, my failing.

In my example I avoided the word student. We know about the student interface as you have outlined Mary...but addendum while Iris does mention student....we don't actually know which age group Iris is working with? if student (FE/HE then Mary-I agree with you about the options you have outlined).
Thus, outside the student seeing feedback/being involved in the process, when midwives, for example and early years teachers collate data about obs they share it in a record book face-to-face with the carer.....the baby/young child doesn't need to log in! And if say a midwife has 50 babies to monitor.....and record accurate obs during their training course that involves their teacher using moodle with them ...then I hope my post makes sense now.
Thus, when Iris says only teacher view (my emphasis: practitioner view).....then that is the need.
Does this marry up with assignment? suppose so...in some sort of ,arguably, convoluted way outside student mode-perhaps
Does this need marry up better with gradebook? =possibly
Does this need marry up much better with import outcomes ......what do you think Mary?

I set it as an hidden assignment

it is an opinion and not supposed to be part of the average of the course
