Using a screen reader I and Web Accessibility toolbar I noticed For= and ID= for Exam Boxes do not match identically for accessibility example label says says Label for="Edit-q56" and ID is "q56". If these could match it would improve accessibility.
Also there is an <H3> for Survey over the question numbers that have <H2> headings. The <H2> should be placed on Survey and the <H3>'s on the numbers.
