Has anyone in medical or other clinical education used moodle for students to log patient contact and/or create a clinical portfolio?
Yes, there are several Moodle tools you can use for students taking a clinical course.
1. Use a Moodle Journal, see plugin https://moodle.org/plugins/view.php?plugin=mod_journal. By default a Journal is private.
2. Use an "individual" wiki (individual is an option in the wiki setting). Each student will have a private wiki. I've used this method and embedded a form in each wiki for students to use.
3. You could use a Moodle blog. However privacy settings on this are global for the entire site. I believe it is too easy for someone to make a privacy change and open up a confidential blog to the entire site or world.

You can try the Dataform module. There is a bit of learning curve due to its high flexibility. You can create a structured log (entry) form in multiple views. You can set it to separate participants so that any user can see only his/her entry. You can assign further viewers to particular entries if needed. You can populate the content to other areas of your moodle while preserving access permissions to content.

Moodle Standard Database Module can do what you want too.
Just create 2 Databases: one for client info, one for files. Then link them together with ONE unique field, like user ID.
I used the Moodle Database Module for CRM (Client Record Management) in several sites.
