Database is a very powerful, stable tool for teaching and learning. But is not good for application form.
The Feedback mod is designed for collect information from current users AND non-current users.
It is good and easy to use for prospective students.
Moodle hide the Feedback mod as default. You have to unhide it as admin.
You must add the feedback mod at front page to allow non-user to access.
