I'm a Moodle admin on our hospital intranet.
In the last two weeks or so we have started experiencing "page cannot be displayed... problems connecting" errors within Moodle, especially during quizzes. This has only happened to me once when I opened up a quiz with 19 questions in it, but other users say that it could happen to them 20 times during the course of taking a quiz. It also happens sporadically throughout Moodle even when not taking a quiz.
What seems to be happening is the browser says "connecting to iAccess" but takes too long to connect and fails.
We had not made any changes to Moodle at the time of the failures, to my knowledge. It occurred at the same time as other weird network issues, but those issues have since resolved themselves. Moodle remains.
Can someone offer some tips as to where to begin troubleshooting these issues? After the problems we increased RAM from 2GB to 4GB on our Ubuntu 12.04 server and restarted the server. We have up to 300 users, but all would have only sporadic usage. We have 50% harddrive usage and around 40% memory usage except for backup spikes once daily. Swap usage 0%.
I am working towards backing up our database and doing a table optimization. We are on Moodle 2.3 and I am working on upgrading to Moodle 2.6.
What else should I be doing to try to resolve this? It is very sporadic but I would probably know within a couple days if users still had issues after making a change.
