When upgrading one of my installations from 1.3 to 1.5+ I receive the following error...
activity_modules block could not be added to the block list
Now, I cannot get any of my blocks to appear. How can I correct this?

For some reason, mdl_block_instance was not being created on the upgrade. When I created the table manually through PHPMyAdmin everything worked fine.
