We have the exact same problem with Moodle 2.5.8 and Captivate 7.0.1.237.
Sporadic grades of "0" for maybe 10% of learners for all our Captivate Quiz. It happens to often now that some learners are showing print screens of the actual grade they received to prove that the reports are wrong. The actual attempt is being recorded, just not the grade. If they take the quiz again, it will sometimes work, other times not..
Here are my options:

For our next quiz, we might publish it to not open in a new window as
it's possible to reproduce a similar problem if a learner completes a quiz,
minimize the window and logs out of Moodle without closing the quiz window's
first.
Any other options we could try?
