Module 'questionnaire' is not readable - check permissions

I have installed/uninstalled/reinstalled the questionarire moddle about 10 times and continue to het this error.  I opened up all permissions on the files on my server, but still am getting the error.  Any advice would be most appreciated.

Never seen that error before...

When do you see it? What actions are you taking?

Seeing it at the very top of the moodle page...in Red. 

Actions to fix..uninstall/reinstall and change perissions on server...all to no avil.

Sorry. What Moodle page are you seeing this at? The admin page?

Did you check it out from CVS? Or download a zip?

Make sure you can see the complete module in your '[moodleroot]/mod/questionnaire' directory.

Do you use MySQL?

I downlaoded the zip file, uploaded it to my server (in the correct /mod/ folder)...unzipped it and things appear fine....that is the permissions look the same as all the other "mod" folders.  So things are fine up to here.  My server does use MySQl...i believe.

I checked and I do use MySQl.  I am also using moodle 1.4.5..any other ideas?

Sorry. I have to ask you this again.

What is the exact page you are on when you get the error message? (what url is showing in the box?). And, what is the "exact" error message?

The reason I'm asking is that I've searched the entire Moodle code and I cannot find an instance of the error message you posted.

Thanks for all the help, but I fixed it.  What I was doing was uploading the zip file to:
/moodle-1.4.5/moodle/mod/

and unzipping it from there (on the server).  The problem was that my server was creating a subdirectory(the default)..that looked like:
/moodle-1.4.5/moodle/mod/questionnaire/questionnaire

A BAD thing, as I needed...
/moodle-1.4.5/moodle/mod/questionnaire

I fixed it and things are working GREAT!
