We have been trialling Moodle 2 with staff and students and would like to get feedback from both. I have created a questionnaire for students easily, but need some advice or guidance on a feedback form for teachers (i.e: first impressions of Moodle 2, theme quality etc)
Does anyone out there have any templates etc I could use or see so I can put my own form together?

Gareth- when you say you have been trialling "Moodle 2" do you mean that the college used Moodle 1.9 previously and so you want feedback on the newer version, or do you mean you are trialling Moodle with them for the first time? I ask because that makes a difference to the kind of questions you might ask, or how you might phrase them.

We have used Moodle 1.x since 2005 all the way up to 1.9.19 but we have moved to piloting Moodle 2 this year with roll out to commence in September 2014. I would like to get some staff perspective on Moodle 2 but just need an idea on the right way to approach them!

Well - what are you trying to achieve with your questionnaire? I mean - presumably you are going to have Moodle 2 whatever they all say in the survey? So is it more to find out which areas you need to offer more support in , rather than what they like/don't like? For example, if they are coming from 1.9 they will find file handling completely different and could potentially be negative about it, but perhaps you can lead the question by asking their opinions of the new possibilities of bringing in file from outside (as opposed to the one upload option in 1.9) As for the theme, I suppose that is your own custom theme? Will it be able to be tweaked according to any comments? I would also ask them how they find it in different browsers and devices, because they might well be accessing it in i-devices on Safari or browsers other than IE (hopefully) and that might affect their view and experience.
