Installing and upgrading help

1.9 --> The URLs were a resource "file" in the mdl_resource table. Module resource

2.0 or greater  --> The URLs are items in the new mdl_url table. Module url

The upgrade from 1.9 to 2.x and to 3.0 went wrong and still having the URLs at mdl_resource and mdl_resource_old tables, pending upgrading. The other resources were correctly upgraded.

It's the only place that I found this problem. I thought that perhaps the @Jon_Jack found a solution.
