Have you ever considered using a wiki in order for your students to buils the questionare that you want? What questions do they think would arrive at the results that you wanted? Maybe break your class up into "teams" and then let them have a go at it. Remember, teaching isn't always giving the correct answer, it's directing students to arrive at the correct answer. And if they do it as a group, all the better.
