There is one student attempted the quiz and gets 72/75, but when we review the quiz there is nothing being answered.
Just a day before the same student attended an exam, she took 2 hours but got 0. She is a good student and always have good marks.
The issue is only happened to this one student. We use Moodle 1.9.

I am wondering which table stores the actual attempts answer?
If this is only happening with one student, I would suspect the following:
1. She never actually submitted the quiz - she might been through and answered everything but didn't actually submit.
2. She never actually took the quiz - have you checked the logs? Even good students goof sometime and then try to cover their tracks!
Or there could be something going with the student's permissions - I would try logging in as that student and seeing if you can submit the quiz and have the same results or if it goes through.
