Your problem is caused by the fact the editor you used to create your file has included a byte order mark at the beginning of the file.
Use a text editor that permit you to create UTF-8 files without byte order mark and the first question name will be taken into account.
I personally use Notepad++, an excellent and free text editor that easily permit to choose encoding and also choose to add or not a byte order mark, but there are many other free editors available.
And to explain what Sakshi saw, as of some months ago, we fixed Moodle to ingore the byte-order-mark if present, so it no longer causes a problem.
