Configurable reports block (plugin)

Hi, My IT colleague tried to install this plugin yesterday for Moodle version 2.3 but we got an "error reading from database" message on every page. 

When I tried to add the block onto my front page using "Add a block" it only appeared in the drop down list as blockname and in the plugin overview section under site administration its listed as shown below;

Can anyone advise me why this is?

We're using PCs with Windows Server 2008 and IE 8.
