I wasn't sure if you needed the short answer or the essay type of answer for your question. I did this example with the short answer.
I used the Description when chosing the place to put multiple questions. It's not really a question but a place to put more instructions, text to read, etc. I then added short answer questions after that. Each short answer was called Answer 1 or Answer 2 etc. Then the next set of questions would go into another Description section.
When setting up this quiz, make sure the edit settings page has selected that the questions will be in the same order as on the preview page and how many questions you want on a page properly set. Also make sure that shuffling questions is turned off in the edit settings page.
With short answer questions, if they are relatively short answers that only specifics will be accepted, they can be typed as answers and Moodle will grade them. If you need to have the teacher grade them and they need to be longer, you will want to use the essay question which is manually graded. This can also be used with multiple choice answers.
