The matrix view has a general purpose section and a repeated entry section.
The general purpose section is for view tags such as the views/filters menus and the quick search/paging. By default the template also contains the ##addnewentry## tag that is the link to create a new entry. In addition to view tags you can put in that section any other content you desire, including images, video and what not. You should not enter there field tags (until further notice) b/c they will not be processed (as you can see in your screenshot ##author:name## and ##time...##.
The repeaed entry section is for field tags. The content of that section will be processed for each entry in the Dataform. By default the matrix view generates a template where standard tags for all the existing fields + the ##edit## and ##delete## tags are arranged in a table. You can add to the section any other content and it will be displayed as added. So for instance, if you want the author's name at the top of the form as a heading you should add the tag ##author:name## right above the table and apply a heading style to it. The ##timecreated## should also be in this section.
Now, the standard field tags are the field names in square brackets. So for instance, for the first item in your form you can create a radiobutton field called 'option' with the options
A - individual
B - OSAA
and then the tag would be [[option] ] and should display radio buttons on edit.
Keep the field names short for readability.
