Is this module only for Moodle 2.3?
We are currently using 2.1 version. And I want to have five survey-like questions with strongly agree, agree, disagree, and strongly disagree type of responses.
My question is I cannot find a setting that allows me to display five survey-like questions and choices. Please let me know what I miss here.

@Sami, you are asking 2 different questions.
Q1: The questionnaire module is available for all versions of moodle from 1.9 to 2.3.
Q2: Please read the Questionnaire documentation. You have to use the rate question type.

I am glad to know that I can use questionnaire in our Moodle 2.1
I read the documentation. The thing is when I turn on editing, there is no drop-down menu or select public for the questionaire type.
I only have timing, response options, content options, and common module setting. How can I select to use the rate question type?
Could you let me know what I miss here?
