Hi , I had post a sample question in StackOverFlow.

I would like to create survey for feedback about course and teacher. Moodle has 1 tool for collecting feedback from your students : Surveys, but Surveys are a set of predetermined questions already developed by the Moodle developers.

 Moodle now has a very interesting activity called Feedback

and it is highly recommended  for your purpose . . take a look ;)

Of course it is in Moodle 2.2 and Moodle 2.3

In addition to the Feedback module, there are also Questionnaire and Choice modules. Just cross your fingers when Survey 2 is released, which hopefully combines the three of them gracefully.

I don't know why the survey module in moodle 2 have a lot of  predefined questions but can not modify by users. In old version we can edit or add new.

Why developers made it more inconvenience for end users ??
