Assign roles in System page Empty

I've upgraded to  1.9.16+ but I've been having problesm lately.

After a few day ago My admin account suddenly became a authenticated user an can't access the admin panel, after  a few searches I was able to bring it back. Now I have another problem after accessing it the Assign Roles in System is Blank.

How can I bring this back?

It's not blank because of missing files, I installed the whole moodle from scratch ver 1.9.16+ (Build: 20120112).

Attached it the screenshot of the problem.

I've installed it for a few months now and it was running smooth all of a sudden this happens.
