I think that the mention of ClamAV is probably slightly mis-directing. The upload of files is not handled by ClamAV, but primarily by the file manager in moodle. ClamAV is an open source virus scanner. After a file is uploaded, it is passed to clamav for virus scanning, before being accepted into Moodle.
What are you actually trying to achieve? Which pages are you trying to limit the file extensions you accept and under what circumstances? The ways that you could do this would depend on what you're actually trying to achieve.
