Hi,
I have setup a questionnaire as a template (Learner Satisfaction Survey Template) and copied the same (changing the module title alone) for all the modules.
I receive response from the learners with the Subject Name as Learner Satisfaction Survey Template Copy ## instead of the respective Module title.
Please look at the attached screen-shot and suggest a solution.
Which version of Moodle are you using?

Moodle 1.9.3 (Build: 20081015) - This is the version currently being used.

Hello,
When you created your new questionnaires in the other courses on your moodle site, using the original Template, and entered a new name, I see no reason for the problem you mention. See attached screenshot.

Sorry, but I'm afraid support for those very old versions of Moodle (and Questionnaire) has ended. Please consider updating to more current versions.
