I think that what moodle needs most is improved functionality in the existing modules (e.g "activity linking,") but that would require a changes to Moodle core libraries, and require a lot of liason with the developers, and good English skills.
So a new module or new block is the way to go.
These would be my choice of new modules
a meet-o-matic block or module.
a dropcash.com type module for accumulating funds for Moodle projects.
a choice/survey/poll module that mobile phones can use.
a moblog block that can accept imode photos.
a "now listening to" block so that students can listen to music as they study.
a "virtual economy module" where people can download as much or as little as their uploads are downloaded (like filepile.org, istockphoto.com)
an experts question and answer block like proz.com's kudoz or experts exchange.
a whiteboard module, using flash
a postit block/funtionality so that students can leave messages to each other on the course page
a hotornot.com block with the user photos!
