how can i know that Clam is running correctly with moodle?

You can use the EICAR test file to test anti-virus software. It's a completely harmless text file, but anti-virus software is designed to pick it up and treat it as if it's a virus.
You can find out more about it at http://eicar.org/86-0-Intended-use.html
If your antivirus software blocks the download, just create a blank file in Notepad and save the following in it:
X5O!P%@AP[4\PZX54(P^)7CC)7}$EICAR-STANDARD-ANTIVIRUS-TEST-FILE!$H+H*
Make sure it ends with H+H* and there's no spaces or new line characters at the end.

i tested and Moodle show me a message about this infected file.
but what about if the file is not infected, Does Moodle show another message to inform user that the uploaded file was successufly tested by clamav?
