Moodle 2.1 - Feedback doesn't work when created on frontpage

I think this is a new problem.  Apologies if it's not.  I've created a new feedback today within the main menu block of the frontpage of Moodle 2.1 (it was a fresh install at 2.0 not an upgrade).

Everything seems to be fine but I don't have an "Answer the questions" prompt to go to when I click on the link.  There is a preview icon next to the feedback name on that page but it just gives me a php error relating to roles and permissions.

I then went and tested the feedback module by creating one within a course and it worked fine.

My final thought was to change to the standard theme in case it was our custom theme causing the problem but no joy there either.

Has anyone else come across this?

Yes, I just encountered the same problem - feedback works in a course but not on the front page.
