Quiz reports correct score but all questions show as correct?

I have a quiz that four people took at the same time.

All four attempts were completed.

All four results recorded.

The highest score was 87% and there are 100 questions.

Three of the four attempts show the correct and incorrect responses.

One of them however (87%) shows 87/100 but when I go through all of the questions, they are all marked as correct, and it shows all of the question responses as correct.

There are no wrong answers displayed, yet the result does show 87/100 which means 13 questions should show as incorrect.

Any idea what might be happening?

It might be that you have the quiz set to adaptive mode with penalties. If that's the case, they may be able to see that they got it wrong and take another go at that particular question(s) and got them right then. So it would show correct, but also show that there was more than one try, with a 10% penalty for each wrong attempt. Or it could be something else. I'm guessing it's the former.
