I'm trying to get to the bottom of an issue that several users are reporting on our Moodle instance. I've done a lot of reading both through Google and the Moodle forums and nothing seems to remedy the problem. This issue is VERY important to us because it's preventing some people from completing the course and receiving a certification. So far, only a handful of people have reported the issue, but we'd like to know what to do in case it starts happening more frequently.
The issue occurs when the user's in question try to go to a specific course view page. We have completion tracking enabled on this course to force the user to go through the activities in a specific order. The problem is, when these users go to view the page, they get an error that states: "Found more than one record in fetch() !"
I looked through just about every table in the database, and the only thing I could find in regards to duplicate records existed in the 'course_modules_completion' table. Both of the users that have reported problems had duplicate records for one of the activities in the course. I tried deleting one of those records for one of the users and logging in as him, but I still got the error. Could this be where the problem lies and, if so, how do I go about resolving it so the user can view the course again?
