You might try this in a clean course and see if you're having the same problem. If not, then I'd suggest creating a backup of the course without any student information and restoring it to a new course name and see if that fixes your problem.
If not, you might want to turn on debugging to see if you can see any errors that might be causing the problem and report the results back here.
