One of our teachers at our school just started getting the error:
Continue: Attempt not inserted
He just started getting this last week with his courses when students are trying to do a lesson. I have been researching the problem and here is what I have found/know.
- I looked at this thread http://moodle.org/mod/forum/discuss.php?d=60109 but this had a ticket that was completed with a fix in version 1.7 something. We are running 1.9.5
- The lessons are not new or something the teacher just created. When it happened with the current lesson we then went back to previous lessons from a couple of months ago and they are now getting the same error when they were not orginally.
- There were no updates or anything unusual that happened to the server in the past week when this started.
- Quizzes and other items are functioning fine. So I know that items can be inserted into our mysql database.
- There is not a size issue with our server running moodle.
I did not orginally configure or set up our moodle server but am now in charge of it. After 2 days of working on this I am hoping I am missing something small. Any suggestions?
