I'm not getting Moodle's gradebook (1.9.5) to calculate as I THINK I have it set up to...
Here's a simple case that reproduces the problem (see screenshot):
- Set the course to Weighted mean of grades
- Add a category with a weight of 10
- Add a grade item to that category, with a maximum grade of 4
(Notice the item automatically has "Extra credit" checked)
- Add grades (some 3's, some 4's) for that grade item and update
- Item average is calculated correctly
- BUT, every student gets 10 (max score) for the category & 100 for the course total
If I un-check "Extra credit", then NO calculations appear in the Aggregation columns for either the category or the course.

I had to think about this for a while since its been a couple of terms since we used Moodle's out-of-the-box gradebook without mods.
I believe you'll find the problem lies in the category total (editable). Without going into a long diatribe on the evils of maintaining inaccurate (IMO) category totals (mentioned prolifically by myself throughout this forum) my assessment is your category total either needs to be manually set to the actual total available for the category (4) or left at 100.
