I've noticed with some versions of Moodle, some older versions, occasionally, when I have gone to Course administration>Users>Enrolled users to manually enrol a bunch of students, I have accidentally enrolled them with the role "None" instead of the role "student" because that is what came up as default and I wasn't paying enough attention to alter it.
