While creating or editing a questionnaire, choosing one of the four specified themes (Default,mystic,crawcram,template) doesn't affect the display of the questionnaire after being activated, is there any settings required?

Make sure you have the most recent version of the questionnaire. There was an update recently that fixed this issue.
