Printing Grades with assignment name and date

I need to have a print out of the name of the assignment or quiz that includes the date it was taken and the score (percentage).  I can't figure this out. Please Help!!!

Hi Penny,
if you need dates, you can only do that from Quiz or Assignment interface.
