Most probably you have not set up "Identify user by" area (displayed on the Import Preview screen)
By default "Map from" is set to First Name, and "Map to" to userid. Change both dropdowns to username and it should work.

There is not an option to choose username from "Map from." I can choose Surname, ID number, Institution, etc, but the combinations I have tried so far have failed. Do you have any other suggestions?

ops, yes, it is our own modifications 
You can try to map from Id Number to "useridnumber" (assuming that your users have ID number fields filled in their profiles)
You can also try Email Address to "useremail" (probably the easiest)
