If I have to add some custom fields into Moodle core tables I usually use some prefix for them, typically abbreviation of the site name or similar (or xxxhack_columnname if you are really paranoid - I hope standard Moodle would never have such column name). Quite recently I was working on a Moodle customization for a client and I was adding fields for the course cost in yet another currency (so the course can have the cost defined in both Czech Koruna and EURO). I found using /local/db (see Development:Local_customisation) very useful for this sort of thing. And tracking changes in git (see Development:Tracking_Moodle_CVS_with_git ), of course
