Sorry for the long delay. I was busy implementing the courses and didn't get a chance to try PHP5 until a few days ago.
You are right - going to PHP5 seems to have fixed the problem with the manager notification checkboxes. I can now clear those boxes, and it retains the value. (It also grays-out the "notice for manager" boxes, which it did not do before.)
