I recently updated to the 2008080904 version of the certificate and it seemed to break the display of the certificate/index.php page. Specifically, the function certificate_get_issue that is called on line 81 could not be found.
I fixed the problem by adding that function (certificate_get_issue) from the previous version of the certificate module lib.php file back into the current lib.php file. I don't know if the function was inadvertently removed or if you intended to accomplish the same functionality through a different method, but I did want to point this out.

I had a similar experience.
It looks like there are two functions with similar names in older versions of the \mod\certificate\lib.php file:

When I added the certificate_get_issue() function from an older version of the lib.php file (v 22.214.171.124 2008/08/09 23:42:59 crbusch) to the current version of lib.php the problem seemed to be fixed.
