Want to replicate a standardized test answer sheet on Moodle

I have just started to use Moodle, so I need your guidance.

I am trying to replicate an answer sheet of a standarized test (TOEIC, to be precise) on Moodle so that I can analyze students' answers later. It should look like this.

1. (a) (b) (c) (d)

2. (a) (b) (c) (d)

3. (a) (b) (c) (d)

4. (a) (b) (c) (d)

I have tried GIFT_Maker VERSION 2 by Timothy Takemoto, but as I need all the choices in each question alphabetically ordered, I need something else.

If possible, I would like to work on Excel first as I have to prepare 100 questions and answers. I have a text editor which can save files in utf-8.
