On the report, the assignments are not in the same order as they appear when viewing assignments? Any thoughts? There's an assignment which comes in the middle of the semester and it's appearing first in the report?

The gradebook is independent of the course layout.
You can change the order of the grade report using the "category and items" page in the gradebook.
