Certificate is generated no matter if you passed the quiz or not

I created a quiz and a certificate that should be generated if the required grade is 100%. Unfortunately the certificate is generated any time and the grade is displayed on the certificate ( 0,00%- if it's failed or 100% if it's passed). Next, if I want to view issued certificate there is nothing, no data.

Could anybody suggest me what I should do to resolve this problem?

Since I've had no reply to my problem, can anybody tell me only if your certificates are generated according to required grade correctly, please. I wonder if it's a problem with storing my data into database (some of my configuration is not correct) or it's a general problem.)
