We use the questionnaire module regularly for large surveys (around 1000 forms with about 70 questions each. I works works well.

I find it a good tool until 1.6 but with 1.7
1. csv export shows no answer in the csv file, only the user and course information.
2. view single entry ist empty

We've run into a problem in our first survey. It seems that the "Total" column does not add the results properly in a Likert Scale. However, when we export to .csv and open in Excel, results match what we see on single user feedback screens. Any ideas?

Hey all, I have just downloaded moodle and wanted to add the quastionnaire module to it.
In the README it is written: "2- Visit your admin page to create all of the necessary data tables"
I visited the admin page by going to localhost and entering the username and password of the admin, but could not know how to add the data tables?

This module is missing the db/mssql.sql file, so it cannot currently be installed with MS SQL.

Don't try an admin page, but an admin directory-
On the moodle server-

This ran the table strcuture compnonet for me.

OK downloaded questionnaire...and I don't follow the earlier email strings on how to get this module to read in Moodle.  Can anyone help?  I put the plug in into my moodle/mod folder then go to Moodle site and admin block and the error reads: Module "questionnaire" is not readable - check permissions

Does anyone know how to make the questionnaire available for guests? I'd like people to answer without having to register. The course is open for guests, but the questionnire says no.

I can't print my questionnaire! Can any one help me?
When I try to print, only a blank page appears...even if I try to print only a selection of the text, or when I try to copy the text, nothing happens!

Will the XML-export patch be updated for  1.9?

Have anybody noticed when you use a questiontype of date, it's off by a day. But in the database its okay. Is there a way to fix that.
For example I made a questionnaire with the question to enter today's date, so I enter the date and then go view by response and the day if off by 1 day.

I'm probing latest questionnaire with moodle19p, and I trying to add some funcionality to the anonymous surveys. The Idea is to permit show/submit anonymous Questionnaires to the guest participant without have to change the configuration of course to allow anoymous access. Even that the survey aren't active for the guest user. I dont want to use third part of another php web applicaton suchs a Limesurvey, etc.

¿Are rols enought power to permit this change? Or perhaps there is another trick to permit this behavior.

I installed "latest version" in 1.9.2

It seemd to work from within a course without groups.

In a course with groups I get the following message:

Fatal error: Call to undefined function groups_get_groups() in /home/bonhoeffer/domains/bonhoeffer.nl/public_html/elo/blocks/quickmail/email.php on line 231

How can I change rating scale start number?

Hi together,

when I use the Questionaire-plugin, it's easy to define the scale for a rating by giving the maximum number of it, e.g. 11. But the result is a scale always starting with 1 (e.g. 1 to 11).
Is it possible to change the scale to start with 0?

This would be much more suggestive, according to

0: completely wrong
5: I don't know
10: completely right

I just installed the questionnaire, it works and I like the options. But for some reason the code is included above each question when I preview or access the questionnaire as a teacher or student. Does anyone know how I can eliminate this? It makes the questionnaire not very user-friendly.

Just a thought, but are you using 1.9 of the module with 1.9 of Moodle? If so, I have managed to get it working but experienced a slight issue with my theme. Do you get the same issue when using a different Moodle theme?

This is a guess as I have only just started to use the module myself.

We are running Questionnaire 2007120102 on Moodle 1.9. It's working great, except for the "Download in text format" button in IE. This works fine on Firefox/Safari but in IE we get an error that says:

Internet Explorer cannot download report.php from <MY_MOODLE_URL>

Like some of you did, I have uploaded this block in the /mod directory for my 1.9 moodle and tried to run the admin page which gave me the following message:

I had the same error, and it turns out I had ./mod/questionnaire/questionnaire/files. You need to remove your current questionnaire directory and replace it with the questionnaire directory within the questionnaire folder (i.e. there is an extra directory level that is unnecessary).

We use the Questionnaire module a great deal for cross college surveys.

We recently upgraded Moodle to 1.9 and the Questionnaire module to the 1.9 compatible version.

In previous versions teachers and admins could view all questionnaire submissions. We're having a problem in that we can only view our own responses.

I've checked the roles and they are as they should be.

Any suggestions?

I am running Moodle 1.7+ with Questionnaire. I had a problem with a corrupted table that could not be repaired. I deleted the tables and attempted to reinstall the module. I will not recreate the table or install. Suggestions please? As usual this is a crisis since courses are scheduled to begin today. Any help would be greatly appreciated.

Is there a way to go in and print all surveys submitted as a batch?

Is there a way to input a score in Questionnaire Plugins? or to add a drop down type of question in Quiz Activity? I tried the Quiz Activity to have a score but I need the drop down type of question in Questionnaire. Please advise me what to do. I appreciate any suggestions to answer my problem. Thank you.

we have the same error on a moodle that is running on https. We have other moodles running on http, where the download of the reports give no problem.

Do you have the same setup as us (https), in other words, can we put that due to the https environment we have this error?

did you find a solution yet?

For those having trouble with downloading the text report via HTTPS using IE:

Find the line "header("Content-Type: text/comma-separated-values");" in report.php and add the following two lines below it:

header('Cache-Control: private');
header('Pragma: ');
